What is Breast Augmentation?
Women choose to enhance their breasts with breast augmentation for many personal reasons. Following pregnancy and breastfeeding, breasts tend to lose their firmness and volume. Some women may feel that their breasts are small and underdeveloped. Other times, breast augmentation is a reconstructive procedure for women who have undergone a mastectomy due to breast cancer.

Implant Options
Size
Breast augmentation implants are measured in cubic centimeters, or cc. Although a more common form of measurement among people is cup size, this is not reliable. A store selling a 34B could be the same size as a 34D in another store.
The Mentor Breast Implant Sizing System
The volume sizing system from Mentor allows patients to achieve desirable results. Along with Dr. Ashruf’s skill and our state-of-the-art center, a visit to our sizing boutique provides our patients with a high standard of care. By utilizing precise sizing and accurate measurements, Dr. Ashruf and the staff at Cosmetic Plastic Surgery of Maryland can provide excellent results. The revolutionary Mentor Sizing System includes 18 unique volume sizers. Each sizer is of a different volume. Patients can try on each one or combinations of the sizers, resulting in a very accurate preview of their procedure result.
Composition
Dr. Ashruf only uses FDA approved saline and silicone implants. A saline implant is inserted into the breast pocket created by Dr. Ashruf as an empty shell and then filled with saline solution to add desired volume. A silicone implant is a quasi-solid silicone gel that is inserted into the breast pocket in its whole form. According to the American Society of Plastic Surgeons (ASPS), over 75% of all breast implant procedures in the world use silicone implants. In the United States, saline implants continue to be popular.
Type
Board certified plastic surgeon Dr. Salman Ashruf uses FDA approved Mentor, Allergan Natrelle, and Sientra Implants.

Shape
Breast implants are created in two different shapes: tear drop and round. Tear drop implants give breasts a more anatomical slope, resembling the natural curvature. Round implants offer a fuller bust as they do not have a slope; this also creates more cleavage.
Surface
Breast implants have a strong silicone shell casing that can be created to feel textured or smooth to the touch. Both tear drop and round implants can be created as a textured implant, but only round implants can be made with a smooth surface because rotation in the breast pocket can create a visible change in shape.
Profile
Breast implant profile is the outward projection of the implants from the chest. There are 3 profiles: low, moderate, and high. The profile chosen for your specific breast augmentation will depend on your frame, body type, and Dr. Ashruf’s skilled recommendation.
Placement
Dr. Ashruf has the ability to place implants either in front or behind the pectoral muscle. The subglandular technique (in front of the pectoral muscle) is ideal for women who have a lot of natural breast tissue which can cover the edges of the implant. The submuscular technique (behind the pectoral muscle) is recommended for women who would need the muscle to cover the implant edges since they have less breast tissue.
Incision
Dr. Ashruf can perform breast augmentation through 1 of 3 incision locations. The periareolar incision is created on the lower border of the areola. This is a great technique for those who desire inconspicuous scarring. The inframammary crease incision is made along the inframammary crease, which is the natural fold made at the base of the breast. This is a good option for women who are undergoing additional breast surgeries as many of them utilize this incision. The transaxillary incision is created in the armpit, otherwise known as axilla. This technique effectively reaches both submuscular and subglandular positioning and makes for discreet scarring.

Procedure
Dr. Ashruf performs breast augmentation procedures with the use of general anesthesia. Typically, the surgery itself lasts one hour. Depending on the patient and the type of breast implant used, an incision is made either under the breast fold, under the arm, or around the nipple. The implant is placed either above or under the chest muscles, depending on the amount of tissue available, and the incisions are closed using sutures or surgical tape.
Out-of-town patients are encouraged to enjoy the local hotel accommodations where Dr. Ashruf or one of our registered nurses can visit to ensure comfort and proper healing. Many patients initially report pain and swelling surrounding the surgical site, but this typically diminishes several weeks after the breast augmentation procedure. Patients are encouraged to rest and relax for one to two weeks, but a full recovery may require up to six weeks.
Complementing Procedures
Breast augmentation can be performed in conjunction with a breast lift for a perkier, full bust. For mothers looking to regain their pre-baby bodies, breast augmentation (with or without a lift) can be combined with procedures such as liposuction and tummy tuck. This combination of breast and body sculpting procedures is called a mommy makeover; it can provide beautiful contoured results. Recovery
Once your breast augmentation procedure is competed, Dr. Ashruf will place you in surgical dressing and a support bra for comfort and to decrease swelling. You will need to have someone drive you home and get you situated for your first night of recovery. Sleeping with your head elevated also helps to counter swelling. Some soreness and tenderness is normal and expected, but can easily be monitored with prescribed pain medication.
You can return to daily activities in 2 – 4 days, but please refrain from strenuous physical activities for at least 4 – 6 weeks. Dr. Ashruf will schedule follow-up visits for you to come back to the office so he can make sure you are healing and recovering as planned.
