What is a Breast Reduction?
Breast reduction surgery—also known as reduction mammoplasty—is a procedure that removes glandular tissue, skin, and fat from breasts, making them lighter, smaller, and firmer. Many women who seek out breast reduction are looking for relief from health problems caused by macromastia (large breasts). Breast reduction provides relief by reducing breast weight and breast size; lifting the position of sagging breasts; and reducing the size of the areola in order to make them more proportionate to the patient’s body.

What to Expect
During a breast reduction, three incisions are made in the breast: one around the areola, one from the areola down to the fold of the skin envelope, and one across the skin envelope under the breast.
The first thing to happen when you arrive as you prepare for surgery is that Dr. Ashruf will mark your incision locations while you are elevated, since your breast shape will become distorted once you are lying down. Once you have been put under the general anesthesia, excess skin and breast tissues are removed and trimmed away from the sites of these incisions. The areola may also be reduced and moved higher on the breast to maintain a natural appearance. The breasts are then sculpted using stitches to close the incisions and are wrapped with supportive gauze.
After recovering from the anesthesia under medical supervision in our surgical facilities, patients can typically return home the same day of the procedure.
Breast reductions are performed using a variety of techniques depending on your personal goals and the specifics of your body.
Typically, Dr. Ashruf removes excess tissue to reduce the size of the breast and reshapes the breast so that it is more appropriately sized. Often times, the size of the areola is also reduced and relocated to the center of the new breast.
After thoroughly evaluating your breasts and your presenting concerns, you and Dr. Ashruf will develop a surgical plan that will include what kind of method will be used in your breast reduction surgery.
The pedicle method is a term that refers to the surgical strategy necessary to maintain nipple blood supply and nerve connection. There are two main types, inferior or superior. The type used will be determined during the consultation and planning stages. The areola may be reduced in size in order to ensure proportionality with your new, smaller breasts. In some cases, liposuction alone can be used to reduce breast size but, more often than not, one of the following methods will be used:
Vertical or Lollipop Incision
Dr. Ashruf will use this incision on those with significant sagging but no need for a large amount of breast tissue removal. The incision mimics a lollipop, as one incision is made around the areola border while the other runs from the base of the areola down to the inframammary fold, or the crease below the bottom of the breast. When the breast and skin tissue are removed and the breasts are now at their desired size, Dr. Ashruf will close the incisions. As an added benefit, the breast will typically become slightly elevated from this procedure, leaving you with not only proportionate breasts but also a more youthful appearance. While this incision will leave you with some scarring, it will all remain below the areola, which is well-hidden when wearing a bra or bathing suit.
Those needing a significant reduction in size and/or those who have lost a lot of weight will typically receive this incision, which is sometimes referred to as the inverted-T breast reduction. Dr. Ashruf makes the shape of an anchor, with one incision made around the border of the areola and a second one running vertically down from the areola base to the breast crease. A third incision is made along this breast crease. The anchor breast reduction scars are virtually the same as those in the vertical reduction, although there will be an additional scar. It’s a thin one that runs along the crease of the breast. With proper post-op care, the scars will significantly fade over time and are easily hidden by clothing.

Breast Reduction Surgery Preparation
You’ll receive customized instructions from Dr. Ashruf during your consultation, and some of these to-do items in preparation of your treatment:
- Stop smoking.
- Cease taking anti-inflammatory medications and herbal supplements two weeks prior to the procedure, as these can increase bleeding.
- Stock your refrigerator, run errands, and pick up any necessary prescriptions, keeping the latter at counter level due to your limited range of motion after to surgery.
- Arrange for a friend or family member to drive you home after surgery, as you will not be able to drive. Have this or another person stay with you that night as well to assist you.
- Wear a loose-fitting shirt that doesn’t need to be pulled over your head.
- Depending on your age and other health factors, you may also be asked to get some medical lab testing done and/or get a baseline mammogram.
Although breast reduction surgery has the highest patient satisfaction rate of all the plastic surgery procedures, some patients seek out ancillary procedures to further enhance results. These can run the gamut from body contouring and additional breast alterations to facial procedures to address a wider range of aesthetic goals.
Dr. Ashruf may perform a breast lift in tandem with the breast reduction since a breast lift can be done using the same breast reduction incisions. Excess skin and breast tissue are removed to give breasts a higher, more youthful proportion, shape, and elevation.
Patients who also desire a leaner, sculpted body may benefit from liposuction. Liposuction is not a weight loss method, but rather, a body contouring procedure that uses suction to remove fat from different parts of the body. It can be performed on arms, chest, chin, back, waist, hips, abdomen, buttocks and thighs, which is why it is often done in combination with other procedures. It is extremely popular, and many patients are pleased with the combination of liposuction with breast reduction.
The tummy tuck procedure involves the removal of excess abdominal skin and the subsequent tightening of the abdominal muscles to create a slimmer, flatter midsection.
Breast Reduction Surgery Recovery
Patients should expect pain around the breasts and incision sites for the first several weeks following the procedure. Patients are encouraged to refrain from heavy lifting and other vigorous activities for up to six weeks following the surgery.
Each patient’s body heals at different rates and it is important to give yourself the proper care to ensure your recovery goes smoothly. Dr. Ashruf and his team will provide you with instructions and answer any questions you have about recovery. In general, most patients take 1-2 weeks off from work and can return to more strenuous activities after about a month. You will be placed in a support bra for several weeks to aid in breasts taking shape to their new, smaller contour.
Any pain and discomfort you experience can be managed with pain medication. Dr. Ashruf recommends walking and light movement the evening of your surgery to improve circulation and healing. The vast majority of breast reduction patients report feeling very pleased with their results and experience improved self-confidence.
