What is Breast Augmentation?

breast-augmentation-marylandWomen choose to enhance their breasts with breast augmentation for many personal reasons. Following pregnancy and breastfeeding, breasts tend to lose their firmness and volume. Some women may feel that their breasts are small and underdeveloped. Other times, breast augmentation is a reconstructive procedure for women who have undergone a mastectomy due to breast cancer.

At Cosmetic Plastic Surgery of Maryland in Annapolis, Dr. Ashruf uses breast implants during breast augmentation procedures to enhance a woman’s breasts. Breast implants are carefully selected to add a full, proportional shape to the breasts that suit each individual patient.

Breast augmentation is the most popular cosmetic procedure among women in the United States, and is one of the most commonly performed procedures by Dr. Ashruf at Cosmetic Plastic Surgery of Maryland.

Candidates

Breast augmentation is a serious cosmetic procedure, and ideal candidates often answer “Yes,” to at least one of these questions:

  • Do you feel your breasts are underdeveloped?
  • Does wearing a swimsuit or tighter top cause you to feel self-conscious?
  • Do you have breasts that are inconsistently sized, where one is larger than the other?
  • Has pregnancy or breastfeeding affected the size and shape of your breasts?
  • Has weight loss affected the size and shape of your breasts?
  • Are clothes often too large around your bustline?

During your initial consultation, Dr. Ashruf will review your medical history and talk to you about your desires for a breast augmentation. Together, you and Dr. Ashruf will decide upon the appropriate size of your breast implants and the ideal type of breast implants, either silicone or saline, for your procedure. Other than the size and type of implant, shape of the implant is also very important in creating a beautiful cosmetic result.

Implant Options

Size

breast-implant-sizeBreast augmentation implants are measured in cubic centimeters, or cc. Although a more common form of measurement among people is cup size, this is not reliable. A store selling a 34B could be the same size as a 34D in another store.

The Mentor Breast Implant Sizing System

The volume sizing system from Mentor allows patients to achieve desirable results. Along with Dr. Ashruf’s skill and our state-of-the-art center, a visit to our sizing boutique provides our patients with a high standard of care. By utilizing precise sizing and accurate measurements, Dr. Ashruf and the staff at Cosmetic Plastic Surgery of Maryland can provide excellent results. The revolutionary Mentor Sizing System includes 18 unique volume sizers. Each sizer is of a different volume. Patients can try on each one or combinations of the sizers, resulting in a very accurate preview of their procedure result.

Composition

blog-implantoptionsDr. Ashruf only uses FDA approved saline and silicone implants. A saline implant is inserted into the breast pocket created by Dr. Ashruf as an empty shell and then filled with saline solution to add desired volume. A silicone implant is a quasi-solid silicone gel that is inserted into the breast pocket in its whole form. According to the American Society of Plastic Surgeons (ASPS), over 75% of all breast implant procedures in the world use silicone implants. In the United States, saline implants continue to be popular.

Type

Board certified plastic surgeon Dr. Salman Ashruf uses FDA approved Mentor, Allergan Natrelle, and Sientra Implants.

We also offer the revolutionary implant sizing system from Mentor, which allows our patients to try on a specific size prior to surgery.

This provides patients with added confidence as they can visualize how their new implants will look on their body. They can also compare the difference between various implant sizes and if they choose, can upload these photos to their photo gallery to share with family, or friends.

Shape

Breast implants are created in two different shapes: tear drop and round. Tear drop implants give breasts a more anatomical slope, resembling the natural curvature. Round implants offer a fuller bust as they do not have a slope; this also creates more cleavage.

Surface

Breast implants have a strong silicone shell casing that can be created to feel textured or smooth to the touch. Both tear drop and round implants can be created as a textured implant, but only round implants can be made with a smooth surface because rotation in the breast pocket can create a visible change in shape.

Profile

Breast implant profile is the outward projection of the implants from the chest. There are 3 profiles: low, moderate, and high. The profile chosen for your specific breast augmentation will depend on your frame, body type, and Dr. Ashruf’s skilled recommendation.

Placement

breast-implant-placementDr. Ashruf has the ability to place implants either in front or behind the pectoral muscle. The subglandular technique (in front of the pectoral muscle) is ideal for women who have a lot of natural breast tissue which can cover the edges of the implant. The submuscular technique (behind the pectoral muscle) is recommended for women who would need the muscle to cover the implant edges since they have less breast tissue.

Incision

breast-augmentation-incisions

Dr. Ashruf can perform breast augmentation through 1 of 3 incision locations. The periareolar incision is created on the lower border of the areola. This is a great technique for those who desire inconspicuous scarring. The inframammary crease incision is made along the inframammary crease, which is the natural fold made at the base of the breast. This is a good option for women who are undergoing additional breast surgeries as many of them utilize this incision. The transaxillary incision is created in the armpit, otherwise known as axilla. This technique effectively reaches both submuscular and subglandular positioning and makes for discreet scarring.

Procedure

breastaugmentationDr. Ashruf performs breast augmentation procedures with the use of general anesthesia. Typically, the surgery itself lasts one hour. Depending on the patient and the type of breast implant used, an incision is made either under the breast fold, under the arm, or around the nipple. The implant is placed either above or under the chest muscles, depending on the amount of tissue available, and the incisions are closed using sutures or surgical tape.

Out-of-town patients are encouraged to enjoy the local hotel accommodations where Dr. Ashruf or one of our registered nurses can visit to ensure comfort and proper healing. Many patients initially report pain and swelling surrounding the surgical site, but this typically diminishes several weeks after the breast augmentation procedure. Patients are encouraged to rest and relax for one to two weeks, but a full recovery may require up to six weeks.

Complementing Procedures

Breast augmentation can be performed in conjunction with a breast lift for a perkier, full bust. For mothers looking to regain their pre-baby bodies, breast augmentation (with or without a lift) can be combined with procedures such as liposuction and tummy tuck. This combination of breast and body sculpting procedures is called a mommy makeover; it can provide beautiful contoured results. For additional information on complementing procedures, please feel free to contact our office.

Frequently Asked Questions About Breast Augmentation

How long after breast augmentation do they look normal?

Although each patient has a different recovery period, it can take up to 3 months for your implants to settle into their “final” appearance. However, they can continue to settle and change beyond 3 months as your body changes, especially if you become pregnant or have changes in weight.

What are the lifting restrictions after breast implants?

It’s recommended that you avoid lifting anything heavier than 5 pounds for the first few weeks after surgery, since this can disrupt your incisions.

How many ccs are in a bra cup size?

Cup size and cc’s are not a direct correlation, so it can be difficult to aim for a specific cup size with implants. In general, anywhere between 130 and 200 cc’s makes up one cup size. It can be more helpful to bring a picture of the breast profile you want to achieve rather than having a specific cup size in mind.

Why do you put breast implants below the muscle?

There are benefits to placing implants below the muscle, like a more natural look and feel, as well as less implant visibility while working out. Some women who don’t have much breast tissue coverage can also benefit from sub-muscular placement.

How do you prevent rippling in breast implants?

Placing the implants below the pectoral muscles is one way to prevent visible rippling. Another method is using fat transfer to create more tissue coverage in the breast itself. Dr. Ashruf can discuss your best options during a consultation.

Do you have to replace a ruptured silicone gel implant and why?

Yes. Ruptured silicone gel implants must always be removed and replaced for your safety. This procedure is sometimes called breast implant revision.

What is the potential for loss of nipple sensation on one side and not the other?

There’s always a chance of losing sensation in the nipples during breast surgery, especially with certain incision patterns. If you’re concerned about nipple sensation, make sure to discuss your options with your plastic surgeon.

How do you make cleavage with breast implants?

It’s important to note that cleavage is not caused by breast size, but rather by clothing. If you want more visible cleavage, your plastic surgeon can discuss your best options for implant size and placement based on your wardrobe and cosmetic goals.

Should I wait until I am done having children to have augmentation?

Breast augmentation will not prevent you from having children or nursing, but you may need a revisional procedure to maintain your results. That’s because pregnancy and nursing can negatively affect your new breast profile.

Will augmentation work for me if I feel my breasts are sagging?

One of the most common combination procedures is a breast augmentation with lift. If sagging is a concern along with low volume, then this can be an effective option. If you choose not to opt for a breast lift, a breast augmentation itself won’t necessarily correct sagging.

How long until I can exercise?

You should be prepared for at least two weeks of avoiding any moderate exercise or movement. After two weeks, you can begin low-impact cardio exercises like walking. After six weeks, you can generally return to your regular regimen, but your plastic surgeon can give you the best indication of when you’re ready.

Will I need additional surgeries?

There’s always a possibility of needing a revisional procedure after breast augmentation, whether because of complications, changing your mind, or unsatisfactory results. Although you may never need an additional surgery, it’s important to be prepared for the possibility of one in the future.

When can I return to work?

You can generally return to office-based work within one or two weeks. If you work a more labor-intensive job, you may need to wait longer. Dr. Ashruf can give you the best idea of when you can return to work.

Will breast augmentation increase my risk of breast cancer?

Breast implants do not increase your risk of breast cancer. However, certain types of implants have been shown to carry a very small risk of a rare type of lymphoma. Your plastic surgeon can help you weigh the risks during your consultation.

Are scars from a breast augmentation noticeable?

This can depend on a lot of factors like your incision pattern, your aftercare, and your wardrobe. There are steps you can take to minimize visible scarring, and your plastic surgeon can recommend your best options.

Can breast implants last 30 years?

Breast implants are created to last about 10-20 years. The older the breast implant is, the higher the risk of rupture or other complications.

Can fat be transferred to breasts?

Yes, fat transfer breast augmentation is a surgical procedure to enhance the shape and size of the breasts using a patients own fat cells.
